Consent Form

The General Data Protection Regulation (GDPR) comes into effect on 25th May 2018.

There are now two distinct privacy policies:

• General and
• Role Holders.

Most church members should use the General option of the Privacy Notice - and fill in a Consent Form.

A signed consent form is required from each General Church Member:
This superceeds any consents you may have given under The Data Protection Act and is available here. Printed copies are available at the back of the Church.

Please return completed and signed forms to Barry. These will be retained on a consent file and this forms part of the way these provisions are administered.
It is important we can demonstrate compliance and this file will be central to that policy. As the Data Protection (Compliance) Officer, Barry will be responsible for keeping it up to date.
